Healthy Together is here for you! Healthy Together is a benefit that can help you and your family manage your health. It is for anyone covered by Wyoming Medicaid, Division of Healthcare Financing (Medicaid or Title 19).
Healthy Together gives you:
- Your own personal Health Coach nurse who will help you make healthy decisions, educate you on your illness and help you find resources in your community including a doctor, dentist or other healthcare provider.
- Educational materials to help you manage your health.
- Meal planning, medication instructions and help with quitting smoking.
- 24 hour 7 days a week nurse line just for you and your family (1-888-545-1710).
Your Health Coach will work with and support your doctor to make sure you are receiving the care and resources you need. Together, we are a team working to help you become and stay healthy.
