Healthy Together
The Healthy Together program was developed by APS Healthcare and the Wyoming Department of Health to assist individuals and families covered by Wyoming Medicaid, Division of Healthcare Financing insurance to establish a healthier and more productive life. Healthy Together works in conjunction with you, the provider, to reinforce your plan of care and provide education, support and resources to your patients.
You, the provider, are a very important part of the Healthy Together team. As a provider:
- Identify, at the point of care, Medicaid patients who will benefit from Healthy Together.
- Promote and refer your patients into the program
- Collaborate with the Healthy Together health coach RNs to create a stronger continuum of care and improved health outcomes.
Healthy Together offers you the provider:
- A team of clinicians (nurses, social workers, licensed counselors, pharmacists and physicians) to help reinforce your treatment plan.
- Support from an after-hours, toll-free education line helping redirect patients from emergency rooms or from after-hours services.
- Free continuing medical education. For more information call 1-888-545-1710.

- Fact sheets on a variety of health and disease topics that you can print from your office and use in your practice with your patients.
- Tools and support to conduct group visits for a variety of health issues to your patients.
- Telehealth technology to improve clinical outcomes for pre-selected clients with chronic conditions with the Health Buddy Project.
- The Partnership Access Line (PAL) is a telephone based child mental health consultation system. The PAL team is available to any primary care provider throughout Wyoming at no cost
